Tuberculosis related esophagomediastinal fistula is rare complication. In a prior study, this condition was improved only by several weeks of antituberculosis medication with non per os(NPO). One rare case of tuberculous esophagomediastinal fistula in a 22-year old man is reported here. He had esophagogastroduodenal endoscopy and diagnosed esophagomediastinal fistula. He was treated with antituberculosis medication and total parenteral nutrition but 4 weeks later the fistula was not closure. In order to avoid surgical procedure we performed percutaneous endoscopic gastrostomy (PEG) and maintain antituberculosis medication and enteral nutrition. 2 months later, the fistula was successfully treated and percutaneous endoscopic gastrostomy (PEG) was removed.
